What hormone replacement therapy does
As estrogen and progesterone decline through perimenopause and menopause, many women experience hot flashes, night sweats, sleep disruption, mood changes, vaginal dryness, and changes in weight and energy. Hormone replacement therapy replaces some of what has been lost, and for the right patient it is the most effective treatment for these symptoms.
HRT is not one thing. Estrogen may be given as a patch, gel, cream or tablet. Progesterone is usually taken as an oral capsule at night, and is required for most women with a uterus who take estrogen. Some women benefit from progesterone alone, particularly for sleep and mood in perimenopause. Bioidentical hormones have the same molecular structure as the hormones your body makes.

The main forms of HRT
Estrogen through the skin (patches, gels and creams) avoids first-pass metabolism in the liver and carries a lower clot risk than oral estrogen, which is why many providers prefer it. Oral micronized progesterone is taken at bedtime and often helps sleep. Vaginal estrogen treats dryness and urinary symptoms locally with very little absorption into the body.

What the evidence says about safety

Do I need lab work for HRT?
