What an HRT patch is
An HRT patch is a small adhesive square that delivers estradiol, the main form of estrogen, steadily through the skin. It is applied to the lower abdomen or buttock and changed once or twice a week. Because the hormone enters the bloodstream directly rather than passing through the digestive system and liver first, patches give steady levels and avoid some of the risks associated with oral estrogen.
Patches treat the classic symptoms of menopause: hot flashes, night sweats, sleep disruption, mood changes and vaginal dryness. Women with a uterus who use an estrogen patch also need progesterone, usually as a nightly oral capsule, to protect the uterine lining.
Patches are one of several transdermal options. Gels and creams deliver estrogen the same way but are applied daily and let the dose be adjusted in smaller steps.
Who patches tend to suit

Patches versus gels, creams and tablets
All transdermal forms share the main advantage over tablets: estrogen bypasses the liver, which is associated with a lower risk of blood clots and less effect on triglycerides and blood pressure. The differences between transdermal forms are practical. Patches are changed once or twice a week and give the most consistent levels, but can irritate skin and occasionally fall off. Gels and creams are applied daily, dry quickly, and allow finer dose adjustment, but require care to avoid transferring hormone to partners or children through skin contact.

Getting started
Regardless of the form, HRT starts with a provider evaluation: your symptoms, your age and time since menopause, and your personal and family history of breast cancer, clots and stroke. From there the provider chooses a form and dose, and adjusts it over the first few months based on how you respond.

How often do you change an HRT patch?
Most estradiol patches are changed twice a week; some are weekly. Your provider or the product label will specify.
Are patches safer than oral HRT?
Transdermal estrogen is associated with a lower risk of blood clots than oral estrogen and has less effect on liver-made proteins. Overall safety still depends on your personal history and on using progesterone when it is needed.
Can I shower or swim with a patch on?
Yes. Patches are designed to stay on through bathing and normal activity. Avoid applying them right after a hot bath or on oily or broken skin.
